Insurance & Authorization

Dr. Fern accepts most insurance plans. Please bring your current insurance cards with you, we will need to make a copy of them. If your plan requires a co-pay, this will be collected at the time of your visit. The billing department will process all insurance forms necessary. Those of you enrolled in a managed care plan or HMO will need prior authorization. Failure to have the required referral/authorization may delay your visit and in some cases, result in having to reschedule your appointment.
